To ensure the best experience for all our patients, we have established the following policies to protect your valuable time and our team’s ability to serve you effectively. We ask that you read and adhere to these guidelines as part of our mutual commitment to your health and well-being.
Activated Health & Wellness
Appointment Policies
1. No-Show Policy
We kindly ask that you respect our time and services by notifying us if you’re unable to attend your appointment. A $100 no-show fee will apply to missed appointments with any provider, and a $200 no-show fee will be charged for missed pellet therapy appointments. For new patients, the deposit will be kept if there is a no-show ($300 for NP/PA appointments and $450 for Dr. Rodarte). These policies help us to provide consistent, quality care to all patients.
2. Late Cancellation Policy
If you need to cancel your appointment, please let us know at least 24 hours in advance. In some cases, a $100 fee may apply to late cancellations, depending on the circumstances. Same-day cancellations are evaluated on a case-by-case basis to ensure flexibility for unique situations while still respecting our team’s schedule.
3. Late Arrival Policy
We understand that life can get busy. If you are running late, we offer a 10-minute grace period for your appointment. We strongly enforce this 10-minute late rule to ensure smooth scheduling for all patients. If you arrive more than 10 minutes past your appointment time, we may need to reschedule. However, we will offer to weigh you in, check if any medication refills are needed, and reschedule the appointment if necessary.
4. Prescription Refills
Please request prescription refills through the patient portal and allow 24-48 hours for processing. This helps us manage refills efficiently while focusing on patient care.
5. New Patient and Pellet Appointments
We kindly request that new patients and those scheduled for pellet therapy arrive 15 minutes early to allow time for check-in and any necessary preparations.
6. Phone Policy
Our team is dedicated to giving undivided attention to in-office patients, so we may not always be able to answer calls immediately. Please feel free to leave a voicemail, as we typically return calls promptly.
7. Communication
To ensure the security of your health information, we use an encrypted patient portal for all communications related to your care. This secure platform keeps your information confidential and HIPAA-compliant. We strongly encourage you to use the portal for all health-related messages. Please note that emails sent from outside the portal are not encrypted and, therefore, do not meet HIPAA standards for privacy and security. For your protection, please avoid sending sensitive information through regular email. Thank you for helping us maintain a secure environment for your health information.
Privacy Policy
We respect your privacy and are committed to safeguarding your personal and medical information. We collect only the necessary information to provide optimal care, such as contact details and medical history. This information is used solely for treatment, appointment scheduling, billing, and necessary communications with other healthcare providers, as required for your care. We will not share your information with third parties without your consent unless required by law.
